Connecting. Growing together. Position Highlights & Primary Responsibilities: Provide daily rounding on patients in skilled nursing facilities (SNF) and long term care (LTC) settings

Serve as the primary provider for one to two SNFs, managing patients across both skilled and long term care levels. Complete an average of 6-9 patient encounters per day, depending on patient complexity, acuity, and the number of facilities covered. The typical combined daily skilled census across one to two SNFs ranges from 8-13 patients

Participate actively in weekly interdisciplinary team meetings at the SNF, as well as departmental and multi specialty meetings. Collaborate closely with Extended Care physicians and the patient's primary care team to align care with patient goals and deliver high quality, value based care.
