Primary Responsibilities: * Oversees and manages the Market Leadership team and ensures assigned market deliverables are implemented in an effective and timely manner. * Develop and maintain a collegial relationship with other APCs and physicians (employed and contracted) to support the business efforts of the organization

Ensures clinical and enterprise communication is disseminated to their healthcare providers in the clinic, region, market in the spirit of transparency across all aspects of our business * Conducts and directs standardized, functional Patient Care Coordination (PCC) meetings and ensures patient information is disseminated to APCs and patient care staff to ensure appropriate patient care is being delivered. * Oversee standardized, functional Patient Care Coordination (PCC) meetings and other clinic specific population management meetings and ensure patient information is disseminated to clinicians to ensure appropriate patient care is being delivered. * Drives impro
